SBDHS is offering a $250 grant for senior dental hygiene students to assist them with their California State Board Examination. You will need to print the grant application and mail it post marked prior to March 27th, 2004.

Adrienne Fowler Memorial Grant Application

The South Bay Dental Hygienists' Society is offering a $250.00 grant to assist a senior dental hygiene student with the cost of the 2004 California State Board Exam.

To be eligible for the grant, the senior must:

1. Be enrolled in an accredited dental hygiene program in Los Angeles County (Cerritos, WLA, USC, & Pasadena).

2. Be a member of SADHA.

3. Have at least a 3.0 GPA.

4. Submit a 250 word essay entitled "What I expect from my local dental hygienists’ society".

5. Attend a SBDHS General (3rd Wednesday) or board meeting (4th Wednesday) before the deadline. (See Newsletter for meeting information)

6. Submit two character references (one from a dental hygiene instructor) with a brief statement (50 words or less) on why the candidate should be considered for this award.

7. The Grant is contingent on the applicant graduating and qualifying for the California Dental Hygiene Boards.

February 15, 2004.
